Please download and follow the directions for the forms below. Bring them, except the read-only ones,  with you to our first meeting. 

• New female clients: Complete 1, 4, 5, 6, 8, 9, 11. Read only 2, 3. (If Medicare-eligible, see below.)
• New male clients: Complete 1, 4, 5, 6, 8, 9, 10, 11. Read only 2, 3. (If Medicare-eligible, see below.)
• Returning clients: Complete 12.  Read and sign 4, 5, 6, 8. Read only 2, 3. (If Medicare-eligible, see below.)
• Medicare-eligible clients: Follow the  directions above pertaining to you; plus sign 7.
